STUDY OF RELATION BETWEEN PLATELET INDICES AND CARDIAC BIOCHEMICAL MARKERS IN PATIENTS WITH CORONARY ARTERY DISEASE AND ACUTE MYOCARDIAL INFARCTION

Abstract :

INTRODUCTION: Coronary artery disease (CAD) is a group of disorders which involves thrombotic lesion in coronary artery. Cardiac markers, troponins (T and I) and the protein or mass of CK–MB and myoglobin are an earlier recognition or exclusion of myocardial necrosis. But both these methods are labour intensive and expensive methods. The present study is an attempt to evaluate relation between platelet indices and cardiac biochemical markers in patients with coronary artery disease and acute myocardial infarction. OBJECTIVES: 1) To study the diagnostic utility of platelet indices and established cardiac biochemical markers Troponin T and CKMB. 2) To study the relation of platelet indices with these cardiac biochemical markers. MATERIALS AND METHODS: After obtaining institutional ethical clearance present case control study was done in RCSMGMC&CPRH, Kolhapur, Maharashtra. Total of 130 such patients of coronary artery diseases and 130 age and gender matched control with non–cardiac complaints were enrolled as study group. Values of platelet parameters (platelet count, MPV, PDW and PLCR) were recorded immediately. Sample for Troponin collected 6 hr and 12 hr and sample for CKMB collected at 4–6 hrs after admission. RESULTS: There were total of 130 patients of CAD which included 30 chronic stable angina, 34 unstable angina and outright myocardial infarction in the form of 16 NSTEMI and 50 STEMI. Most of the cases (n=89, 68.46%) were in 45–65year age group. There were 89 males (68.46%). Cardiac Troponin T as well as CKMB had excellent specificity. In terms of sensitivity of Troponin T was better than CKMB in STEMI cases but in overall MI cases sensitivity of CKMB was better. In the STEMI group 64 % cases (n=32) were Troponin– T positive with MPV ≥ 10.94. In the STEMI group 60 % cases were Troponin– T positive with PDW ≥ 12.05. In the STEMI group 58 % (n=29) were Troponin– T positive with P–LCR ≥ 21.89. The mean value of all platelet parameters (MPV, PDW, P–LCR) were found to be higher in CKMB +ve MI cases than CKMB and in Trop +ve MI cases. CONCLUSION: The platelet indices were raised in both Troponin I and CKMB positive cases but showed statistically significant best relation with CKMB. The case control study of platelet indices in CAD thus showed promising diagnostic utility.
