ROLE OF TAMSULOSIN WITH/WITHOUT DEFLAZACORT IN MANAGEMENT OF LOWER URETERIC CALCULUS LESS THAN 10 MM- A PROSPECTIVE RANDOMIZED COMPARATIVE STUDY

Abstract :

Ureteric stones account for major bulk encountered in urological practice. Urologist’s armamentarium to treat ureteral stones consists of SWL, URS, PCNL laparoscopy and medical expulsion therapy (MET) using alpha blockers, steroids etc. The aim of study was to evaluate efficacy of tamsulosin alone and of tamsulosin and deflazacort combination in the expulsion of lower ureteric stone with size less than 10 mm. A prospective randomized study was conducted of 136 consecutive patients of symptomatic lower ureteric stones of <10 mm. The patients were randomly allocated to two groups – Group A: Tamsulosin, Group B: Tamsulosin + deflazacort and followed for a period of 7 days. The overall expulsion rate was higher (82%) in group B as compared to group A (70%). Larger stones of >6 mm showed higher expulsion rate in group B (p–value 0.032). The mean time of expulsion observed was less in group B (6.2 days) after therapy commencement, as compared to group A (6.9 days). Thus for lower ureteric calculi <10 mm MET is a reasonable therapeutic choice, with better outcome seen with Tamsulosin + deflazacort in increase in stone expulsion rate and reduced expulsion duration
