ROLE OF MINIMALLY INVASIVE FIXATION FOR TWO-PART PROXIMALHUMERAL FRACTURES (SURGICAL NECK) USING J-NAILS

Abstract :

Background– Proximal humeral fracture is one of the common fractures in the elderly population. Non–operative and operative treatment have their own advantages and disadvantages. Hence the present study was conducted to determine the role of minimally invasive fixation using J–NAILS in the surgical neck humerus fracture. Objectives –To study the results of minimally invasive fixation using J–NAILS in the surgical neck humerus fractures and to study the complications if any. Methodology– This non randomized prospective interventional study was done on cases presented with surgical neck humerus fractures. All cases were given initial first aid and were examined thoroughly. All preoperative investigation with Standard anteroposterior, Y–scapular and axillary roentgenogram of the affected site was taken, and the fracture was classified according to NEERS CLASSIFICATION and fixed using J–NAILS. Follow up visits were conducted at 2 weeks after surgery then every 4 weeks till 24th and at 30th week. Outcome was evaluated according to clinical method of functional assessment of the shoulder (Constant and Murley score). Result– The patients had immediate pain relief and early mobilization was started. In this study we observed that minimally invasive technique of elastic fixation of surgical neck humerus fracture could be used as good alternative with minimal complications. Conclusion– Being minimally invasive procedure, J–nail fixation is simple and associated with minimal pain and complications.
