Abstract :

Aim: To compare multiple clinical scoring systems & modified CT severity index for diagnosing in acute pancreatitis. Material and method: This prospective randomized clinical study was conducted among 50 subjects in the Department of General surgery, Muzaffarnagar Medical College & Hospital, Muzaffarnagar, Uttar Pradesh during the period 2017–2019. The physical examination focusing on abdomen and chest was performed. Specific blood tests were done to identify pancreatic inflammation (elevated amylase/ lipase). Imaging tests were done to exclude other diagnosis, confirm the diagnosis of acute pancreatitis and assess the quantitative disease severity based on Ranson�s scoring, PANC3 score, CRP level, HAPS score and MCTSI. Results: In our study population, most sensitive clinical parameter for prediction of prognosis of AP was PANC3score followed by Ranson�s score while CRP level was found to be least sensitive. Most specific clinical parameter was also PANC3 score followed by Ranson�s score and CRP level was found to be least specific. Hence, most accurate clinical parameter for prediction of prognosis in acute pancreatitis was PANC3 followed by Ranson�s score. Conclusion: MCTSI is a good prognostic indicator in predicting the incidence of local complications in acute pancreatitis.
