A study of Fournier's Gangrene and usefulness of Fournier's Gangrene Severity Index in predicting the outcome.

Abstract :

INTRODUCTION : Fournier’s gangrene (FG) is an acute, rapidly progressive and potentially fatal, necrotizing fasciitis of infective etiology affecting the scrotum and penis, perineal and perianal regions. The disease affects all groups of age and is more common in patients with diabetes, chronic alcoholism and immunosuppressive disorder. AIMS & OBJECTIVES : To study the etiology, predisposing factors, microbiology, management & various factors influencing the final outcome associated with FG and usefulness of Fournier’s Gangrene Severity Index(FGSI) in predicting the outcome. METHODS: 38 patients with FG admitted in Deptt. Of Surgery, RIMS, from January 2018 to September 2019 were included in the study. Information regarding etiology, microbiology, management and mortality was studied & analysed RESULT & CONCLUSIONS : The disease most commonly affects male, with average age of patients being 56.5 years. The most common source of infection was from the colorectal area. Diabetes is the most commonly associated comorbid factor (78.95%). The disease is most commonly polymicrobial with E.Coli being the most commonly isolated organism. Mean number of deidement performed was 3. Secondary closure was the commonly performed surgery. Mortality associated with the disease was high (18.4%). FGSI is a useful indicator in predicting mortality. Despite of recent advances the disease still carries significant mortality. Early resuscitation with aggressive deidement is required in most of the cases.
