GIANT CELL TUMOR OF TENDON SHEATH- ATYPICAL PRESENTATION

Abstract :
The giant cell tumor of tendon sheath (localized nodular tenosynovitis) is the second most common tumor involving the hand and is frequently reported in the literature. Giant cell tumor of tendon sheath (GCTTS) is the second most common nonepithelial tumor involving the hands.Its frequency is exceeded only by ganglion cysts.It appears to present more commonly to surgical services. CASE PRESENTATION: A 39-year-old man presented to the surgery opd with a 4-week history of a fast growing painful mass involving his left ring finger. There was history of antecedent trauma. Physical examination demonstrated a firm,lobulated 3.0 X 3 cm mass on the dorsal surface of the proximal phalynx of left ring finger. The mass was not attached to the overlying skin. The initial clinical differential diagnosis included soft tissue tumor, hematoma.Needle aspiration was performed under local anesthesia,no pus or blood found.MRI hand showed Giant cell tumor of tendon sheath.The patient was referred to the plastic surgery service where, under supraclavicular block and tourniquet control of bleeding, complete resection of the mass was performed. Grossly the tumor was a lobulated, firm, yellowish own 3 X 2cm mass.specimen sent for HPE. Microscopic examination confirmed the diagnosis of GCTTS. CONCLUSION: This case has been reported due to its atypical presentation of short duration and rapid growth preceded by a traumatic event. patient recovered well in the post operative period
