Diagnostic performance of transperineal sonographic cervical length measurement for diagnosis of preterm labor: a diagnostic test study

Abstract :

Objective: To evaluate the diagnostic performance of transperineal sonographic cervical length measurement (TPS-CL) compare with transvaginal measurement (TVS-CL). Materials and Methods: The participants were 107 suspected preterm labor women whom cervical dilatation was < 3 cm. TPS-CL and TVS-CL were measured. The short cervical length (CL) was diagnosed when CL < 3 cm. The diagnostic performance of TPS-CL was evaluated. Results: Short CL (CL < 3cm) was found in 52.3% by TVS-CL and 51.4% by TPS-CL. TPS-CL had high correlation with TVS-CL (r = 0.77). The sensitivity, specificity, positive and negative predictive value, and accuracy of TPS-CL were 78.6% (95%CI 65.6-88.4), 78.4% (95%CI 64.7-88.7), 80.0% (95%CI 67.0- 89.6), 76.9%% (95%CI 63.2-87.5), 78.5 % (95%CI 69.5-85.9), respectively. Conclusion: TPS-CL had high correlation with TVS-CL. In case of TVS-CL is unavailable, TPS-CL can be used for preterm labor diagnosis.
